CROWNWeb Information

Detailed Timeline Issued for Transition to Electronic Reporting!

August 6, 2008. With the newly published Conditions for Coverage comes a challenging CMS mandate for all outpatient dialysis facilities and for Network 8. Beginning February 2, 2009, facilities will be required to submit all 2728 and 2746 Forms, all patient events, and all CPM and Fistula First data electronically.

CROWNWeb, a free, secure Internet software program created by CMS, will be the method for submitting data. All facilities will use this software—no more paper forms of these types to send to the Network office!!

In DCI, FMC, and DaVita facilities most of the information about this new process will come from your corporate offices—as will information about training. Some of your required data will be uploaded for you from your corporate computers; some will be manually entered at the facility. You will be receiving additional information from your corporation as the time gets closer.

For those who are in all other facilities, free training will be provided at regional training seminars and via web-based tutorials. This training is especially important because all data will be manually entered at these facilities.

Below is the countdown timeline we can provide as of now. It will probably change somewhat as projects get firmed up so check our website frequently to keep up with the latest news on this project.

August 20, 2008— Ending roster for July 31, 2008 will be sent to facilities along with a 2008 Missing Forms list. Facilities will be requested to update patient addresses so that when your facility roster appears in CROWNWeb, patient addresses will be accurate as of this date.

August 25, 2008— Facility Personnel and Services Survey will be sent to Head Nurses. All certification data, services offered, and current personnel will need to be updated so that this section of your CROWNWEB information will be current as of this date. Facilities will maintain this information in CROWNWeb after February 2009.

October 13-17, 2008—Network 8 Annual Meeting in Nashville—Please join us! We will provide you with any updated CROWNWeb information made available to us at that time.

November 10, 2008—the October PAR MUST be received at the Network

November 20, 2008— last quarterly Patient Roster ever will be sent to facilities. This Roster should be checked very carefully to make sure it is correct. If necessary, corrections can be made on the next PAR.

November – December 2008

User Account Enrollment for CROWNWeb facility users will occur during this time. Although details are still being finalized, we anticipate that each facility should enroll at least two staff for user accounts. This will require completing and notarizing forms that we will send to you. These two staff members will have the ability to create other user accounts for additional staff members at your facility as needed.

Enrollment for CROWNWeb Training will occur during this time. Details are still being finalized for training sessions to be held during January 2009. The dates and locations are listed below. The LDOs will be arranging their own training for their facilities, so this free, hands-on training is primarily for the Independent facilities. It is recommended that each Independent facility plan to send two staff members to the training. These staff members will then be able to train others at their facility who will need to use CROWNWeb.

On-line training will be available to all staff members at all facilities. The on-line training is designed in task-specific modules so that your Data Contact, for example, can learn how to enter new patients but will not need to learn how to enter the clinical data for CPMs or Fistula First.

December 10, 2008—the November PAR MUST be received by the Network

December 15, 2009—The 2008 Annual Survey (Form 2744) will be sent to facilities for balancing through November 30, 2008. Because the Network’s current patient tracking system will be shut down in January to prepare for CROWNWeb, we will need to complete the Annual Survey in December for events through November 2008. Then we will have only December to get balanced at the end of the year. We will be on a very short timeframe to get the Surveys back and balanced before our current computer system is shut down in January.

December 31, 2008

LAST DAY THAT FACILITIES CAN MAIL OR FAX CMS 2728 and 2746 FORMS or submit data via VISION to the Network.

January 1-31, 2009— Facilities must hold all 2728s and 2746s not mailed/faxed by December 31 for entry into CROWNWeb after February 2

February 2, 2009—CROWNWeb goes live nationwide—all data is to be entered by facilities (except VA and Transplant facilities). The Network will play a support role to facilities using CROWNWeb rather than the data entry role as in the past.

CROWNWeb - Coming in 2009!

The Consolidated Renal Operations in a Web Enabled Network (CROWNWeb) project is electronic data-submission software implemented by CMS to improve data consistency, reduce complexity, and provide secured electronic data collection.

The new Conditions for Coverage require that all ESRD facilities submit patient information (events, forms, lab data) to the Networks electronically (instead of on paper.) In February 2009 CROWNWeb will be available to all facilities via an exceptionally secure Internet site. Access will require prior registration, and will include secured HTTP entry, data encryption and decryption, restricted user roles, and audits of all user actions in the system.

In the following months, CMS will continue to share information regarding CROWNWeb, and will be providing the minimum system requirements and information on CROWNWeb training opportunities to those who are interested. This training will be provided with no direct cost to the community.
