CROWNWeb (Consolidated Renal Operations in a Web-enabled Network) is an Internet-based data collection system that it's final phase of testing, referred to as Phase III, on January 17, 2012. This phase will add additional facilities to CROWNWeb and will implement some additional security features, including multi-factor authentication and the deployment of QIMS. A national release of CROWNWeb is planned for March, 2012. At that point all Medicare certified dialysis facilities will use CROWNWeb.

With the national rollout of CROWNWeb, ESRD facilities will be required to submit the following information electronically using CROWNWeb:

  • Form 2728
  • Form 2746
  • All patient events (including starts, transfers, deaths, and changes in modality)
  • All CPM and Fistula First data
  • Annual Facility Survey (Form 2744)

Facilities will no longer submit the information described above on paper forms to Network 8 once CROWNWeb is implemented nationally.

On-line training is provided by ProjectCrownWeb for all staff members at all facilities. The on-line training is designed in task-specific modules. The first step in accessing this training is to create a Learning Management System (LMS) account. This is done through the ProjectCrownWeb site. This free account will allow you to access any of the training modules provided. The content is designed so that the Data Contact from your facility, for example, can learn how to enter new patients while someone else from your facility will learn how to enter the clinical data for CPMs or Fistula First. In addition to the online training modules, simulations are available for a number of typical activities performed in CROWNWeb.

Each facility should use QIMS to enroll a Security Official, an End User Manager, and end users. Facilities that are not participating in Phase III may begin the registration process January 30, 2012. The Security Offical will need to enroll first. After the Security Official's account has been established by the Help Desk, then the End User Manager will begin the enrollment process. The end users at the facility will be able to enroll once the Security Official and End User Manager accounts have been activated.
