Grievance Process

Patients can call the Network office regarding grievances toll-free at (877) 936-9260.

ESRD Networks are authorized by CMS to receive and respond to patient concerns and complaints. Patients who have concerns about their care are encouraged first to utilize the local facility grievance process, since each facility is mandated to offer a problem-resolution process.

In cases where that may not be possible and in cases where that has been tried and the resolution was unsatisfactory, patients may contact the Network 8 office in writing or by using the patient-only toll-free number.

Please describe the situation and let us know if we have permission to disclose your name to the facility when we contact them to discuss your concern. If you prefer to remain anonymous we will not disclose your name to your facility.

To contact the Network by telephone:

Use the patient-only toll-free number: (877) 936-9260. This number is available between the hours of 8:00 a.m. and 5:00 p.m. Central Time, Monday through Friday. After hours calls will be answered by the automated phone system and you may use extension 15 to leave a voice mail message.

How the process works:

For all concerns, you will be contacted in order to better understand the nature of the problem. Depending on the nature of the call, the Network may function as a facilitator, advocate, counselor, or referral agent in order to expedite a resolution of the concern. If the matter seems to involve an immediate health and safety issue, it will be referred immediately to the State Survey Agency in your state, which is the entity authorized to conduct on-site investigation of those matters.

In some cases, a committee that includes an ESRD patient and other ESRD professionals will review the concern and offer advice on methods designed to lead to a satisfactory resolution of the matter. It is our goal to do all we can to assist you in reaching a satisfactory resolution to the matters that concern you.
