Dialysis Patient Information Cards
One of the lessons learned last year from hurricane Katrina is that dialysis patients who travel away from their home units need a minimum amount of information with them at all times for their alternate dialysis facilities, other health care providers, shelters, and law enforcement authorities regulating traffic flow.
A “Dialysis Patient Information Card” was designed with CMS oversight and approval to provide individual patient information on one side and toll-free numbers for large dialysis organizations and southeastern ESRD Networks on the other. Patients should carry this card in their wallet or purse to facilitate their care as a dialysis patient.
For patients who started dialysis or transferred in to your unit after May 31, 2006, or for patients who need a replacement card, click on the link below
to find a copy of the card that you can complete and print out. For the NW8 ID Number on the card, you can call Network 8 and ask for the number or leave it blank.
