Fistula First Facility Toolkit
In collaboration with CMS, the ESRD Networks, and the Institute for Health Care Improvement (IHI), Network 8 has prepared this Fistula First Toolkit to help you better understand and implement the "Fistula First Change Package" in your facility. The change package, which consists of recommendations based on best practices, will foster the goals of the National Vascular Access Improvement Initiative (NVAII), also known as "Fistula First".
The introductory materials include: a project overview, the change package, baseline data, references, and a facility "self-assessment". We hope you will find the various tools and resources helpful in identifying and addressing possible areas of improvement. The protocols and forms included are for your review and use as you see fit. Use of these protocols should be discussed with your medical director and administration before implementing.
